The Aware Epidemic
Disease spreads through contact. Awareness of disease spreads through media and conversation. In most coupled models, awareness reduces infection rate symmetrically: learning about the disease makes you more cautious regardless of your prior beliefs. But real awareness feedback is asymmetric. People who already believe the disease is dangerous become more cautious when they hear about it. People who believe it’s overblown become less cautious — hearing about it triggers reactance, not caution.
Franceschi et al. (arXiv:2603.12041) build a kinetic SIS model with this asymmetric awareness feedback. The population has a continuous opinion variable (how dangerous they believe the disease is) coupled to infection status (susceptible or infected). The epidemic dynamics and opinion dynamics evolve simultaneously, each influencing the other.
The macroscopic limit — derived rigorously from the kinetic equations — reveals polarization as an emergent phenomenon. Even starting from a unimodal opinion distribution (most people roughly agree), the epidemic splits the population into two camps: the highly aware (who reduce their contact rate and avoid infection) and the anti-aware (who increase their contact rate in defiance and drive sustained transmission).
The sustained transmission depends on the anti-aware cluster. If awareness feedback were symmetric, the epidemic would burn through the unaware and die out as awareness rises. With asymmetric feedback, the anti-aware population acts as a persistent reservoir — a group whose behavioral response to epidemic information is the opposite of what public health assumes. The steady state has both camps coexisting, with infection concentrated in the anti-aware group.
The mathematical structure is clean: a Boltzmann-type kinetic equation with a contact rate that depends on opinion, producing a Fokker-Planck equation in the macroscopic limit. The polarization isn’t imposed — it emerges from the asymmetry in how opinions respond to epidemic information.
